Job Description:
The certified Nurse Practitioner (NP-C) is a member of the Transitions and Life Choices team who works in collaboration with physicians and other medical staff to provide Palliative Care services to patients and their families. The NP-C engages in independent decision-making about health care needs and provides health care services to individuals and their families. The NP-C performs a comprehensive history, including chart review and information gathered from the family and performs physical and psychological health assessments. The NP-C provides direct patient care as well as consultative services to assure state of the art Palliative Care. The NP-C balances the workload and resources of the inpatient Transitions and Life Choices team. Job Qualifications:
Minimum Education:Masters in nursing or public health. Minimum License/Certification/Registration: Current MN RN license. NP certification in relevant specialty through the ANCC. Approved by the Minnesota Board of Nursing for prescription writing privileges. Possesses active DEA number. Minimum Experience: Three to five years in inpatient, hospice, or home care.
Preferred Experience:
Three to ten years of experience in Palliative Care or related fields. One to three years as NP-C.
Shift: Days/60hrs
