As our client’s new Case Manager, you’ll be responsible for ensuring that the care coordination provided to patients is appropriate for an inpatient rehab facility, for ensuring that the needs of the patient, family, and payor are met, and for coordinating the discharge planning process. You’ll interpret insurance coverage, communicate care coordination, and manage activities across the continuum of care.
To qualify, you must be a Licensed RN with current licensure in the state of Georgia (BSN preferred), and possess at least three years of Case Management experience in an acute care setting (rehab experience preferred). You must also have comprehensive knowledge of insurance reimbursement with a minimum of one year of case management experience. The successful candidate must receive a CRRN within two years of being hired (our client will reimburse for the certification).
