FT Day Position – Candidate will work 3 days as assessment nurse and 2 days as staff nurse.
Duties will include:
Assume the responsibility to travel to hospitals and other selected venues of healthcare locations to perform a face to face clinical assessment and conduct a medical record review. Based on clinical assessment and review of medical record, forecast clinical needs, specialized equipment needs and reimbursement classification for referrals. Makes recommendation to supervisor regarding appropriateness of referral for admission. Meets with various members of the interdisciplinary team in planning treatment plans and identifying opportunities for improvement to optimize quality, customer satisfaction and achievement of expectations set forth by regulatory an reimbursement bodies. Interpret nursing and clinical reimbursement care policies and procedures to personnel, patients/residents, family members, etc. Participates in admission, discharge planning, conducts clinical patient/resident assessments, completes state/federal mandated resident assessment instruments, develops and implements patient/resident care plans.Performs duties such as completing medical forms, reports clinical reimbursement forms, and clinical documentation in medical records. Assist resident care coordinator in performing all facets of the clinical and documentation system directly or indirectly related to the facility’s operational, reimbursement and regulatory compliance goals. Provides training for new and less experienced personnel.Responsible and accountable for ensuring related patient/resident clinical process and documentation systems in which you have a role are consistent with the policies and procedures of the facility including, but not limited to nursing services, professional standards of practice and regulatory bodies.
Candidates must possess a current license to practice as a RN in the State of Ohio. Must have 3 to 5 years experience with MDS and various reimbursement processes and ability to manage all related aspects. Must have working knowledge of Medicare, Medicaid, ODH and JCAHO rules and regulations governing long-term care.
